Whether an employee is voluntarily leaving employment or involuntarily separated, employers often want to know the employee’s opinions regarding various workplace matters. This is also an opportunity to allow the employee to explain why he/she is leaving (if voluntary) or express workplace concerns (such as unlawful discrimination) that can be addressed if appropriate before the concerns become workplace problems. These forms are for use in such meetings with employees upon resignation or termination.
